Cocaine is a stimulant derived from the leaves of the coca plant. It is usually sold in a white powder or synthesized into crystals or rocks as “cracks.” Snorting, smoking, or injecting the drug elicits a brief euphoric, energizing, and exhilarating feeling that lasts a few minutes to an hour, followed by an unpleasant comedown. Cocaine use disorder is when a person keeps using cocaine despite the cognitive, behavioral, and physical problems it causes.

In normal instances, the brain releases dopamine in response to possible rewards, such as the smell of good food, then cycles back into the cell that released it and shuts off the signal between nerve cells. However, in cocaine use, dopamine is prevented from recycling, which causes extra amounts to build up between nerve cells. Because of this “flood” of dopamine, normal brain communication is interrupted and causing the high that people get from cocaine.

A few combination medications, such as topiramate and mixed amphetamine salts, also appear promising.
